Services Offered by Rural Warwickshire North PCN
Our Primary Care Network (PCN) offers a range of services designed to enhance patient care and support the local community. Below are some of the key roles and services provided through our PCN:
ARRS roles
We provide a range of roles funded through the NHS Additional Roles Reimbursement Scheme (ARRS). These roles are designed to expand and enhance the workforce within our primary care network to improve patient care and access.
Key roles include:
- Clinical Pharmacists
- Social Prescribers
- First Contact Physios
- Mental Health Practitioners
- Paramedics
Enhanced Access
We are committed to providing better access to healthcare services outside of regular working hours. This includes evening and weekend appointments to ensure our patients can receive care at times that are convenient for them. Enhanced access helps reduce waiting times and ensures patients receive the right care when they need it.
First Contact Physiotherapists (FCPs)
Our First Contact Physiotherapists are available to provide immediate assessment and management of musculoskeletal problems without the need for a GP referral. FCPs help patients get quick access to expert care, improving outcomes and reducing GP workload.
Clinical Pharmacists
Our Clinical Pharmacists are integral to our healthcare team, providing expert medication management. They work with patients to review prescriptions, optimize treatments for chronic conditions, and ensure the safe and effective use of medications.
Mental Health Practitioner Services
Our Mental Health Practitioners provide support for individuals experiencing mental health challenges such as depression, anxiety, and stress. They work closely with patients to manage and improve their mental health, ensuring timely access to care and intervention when needed.
Social Prescribers
Our Social Prescribers connect patients to non-medical support services within the community. This includes access to services such as housing support, financial advice, community activities, and more. Social prescribing aims to improve overall well-being by addressing the social factors that influence health.
What is the NHS APP?
The NHS App is owned and run by the NHS. It can be accessed by anyone aged 13 and over registered with an NHS GP. Once you have verified your identity in the app, you will have easy, 24/7 access to a growing range of health services and information.
It doesn’t replace existing services. You can still contact your GP surgery in the usual ways.
